IHH Care Coordinator – AbbeHealth – Cedar Rapids, IA

Abbe Center for Community Mental Health, is a subsidiary of AbbeHealth Services located in Cedar Rapids, Iowa City, and Manchester.

Abbe Center for Community Mental Health is seeking a full or part-time time Care Coordinator for our Integrated Health Home (IHH) program in Cedar Rapids office. Care Coordinators work with a team of professionals to enhance the physical and mental well-being of clients in the IHH program. This position includes Integrated Health Home hours that are office-based and work in client’s homes in the Cedar Rapids area. Monday-Friday daytime hours with possibility of a limited work-from-home schedule and flexibility in hours.

Responsibilities

This full time position is responsible for being well-organized and energetic with exceptional interpersonal and communication skills. Duties include focusing on the whole health of individuals-both physical and mental health, writing assessments of needs and goals, making referrals, coordinating care and working in a team environment.

Qualifications

Requires a Bachelor’s degree in social work or human services related field with a minimum of one year experience working with individuals with mental illness. Hourly rate is based on level of education and years of experience. Availability to work Monday-Friday daytime hours. Must pass records and driving checks, pre-employment drug screen and have a valid Iowa driver’s license.
